„Universal Health Coverage“ ist das aktuelle Schlagwort für die künftige Ausrichtung der globalen Gesundheits- und Entwicklungspolitik nach 2015. Die Lebenserwartung gilt dabei als einer der zentralen Indikatoren. Wenn wir die Auswirkungen von unbehandelten HIV-Infektionen auf die Lebenserwartung weltweit betrachten, scheint es naheliegend, HIV-Prävention und Behandlung als zentrales Instrument zur Verbesserung der Lebenserwartung einzubeziehen.

Ein Fokus auf Universal Health Coverage verlangt zudem nach einem stärkeren Engagement von zivilgesellschaftlichen Partnern, um auch jene Bevölkerungsgruppen zu erreichen, die bisher von der allgemeinen Gesundheitsversorgung ausgeschlossen wurden. Gerade in diesem Bereich haben HIV-Organisationen Wertvolles geleistet und können von Erfolgsgeschichten berichten, die auch für weitere Gesundheitsthemen inspirierend sind.

Samraksha zum Beispiel, eine Nichtregierungsorganisation im südindischen Staat Karnataka, arbeitet seit Jahren mit Gemeinschaften zusammen, die besonders benachteiligt sind: Sexarbeiterinnen, Transmenschen, Frauen und Familien, die von HIV betroffen sind. Samraksha hat zahlreiche HIV-Interventionen initiiert wie Beratungen vor und nach dem HIV-Test oder ein Pflegezentrum für HIV-positive Menschen und war Geburtshelferin für zahlreiche heute unabhängige Organisationen HIV-positiver Menschen.

„Die erschreckende Ungerechtigkeit und soziale Marginalisierung, mit denen die betroffenen Menschen konfrontiert wurden, führte dazu, dass wir uns konsequent einem Menschenrechtsansatz verpflichteten“, erklärte Sanghamitra Iyengar, Gründerin und Direktorin von Samraskha. „Wenn sich ihr Leben tatsächlich verändern soll, muss die Gesellschaft sie anders ansehen und sie als Menschen zu respektieren."

This approach, in essence, tells us something about the sad situation we have come to live in. A reality in which the financial oligarchy have taken over country democracies, according to Simon Johnson’s The Quiet Coup. A reality in which economic inequalities have an enormous negative impact on health equity and social wellbeing. "On Jan 15—17, 2013, over 800 experts in maternal health came together in Arusha, Tanzania, to present the latest evidence on improving the quality of care for women during pregnancy and childbirth. The past 25 years of the safe motherhood movement have seen extraordinary successes—notably a 33% reduction in maternal mortality from 409'053 in 1990 to 273'465 in 2011. These achievements have motivated and mobilised a welcome new generation of political and financial commitments to maternal health. But with the era of the Millennium Development Goals (MDGs) drawing to a close in 2015, a moment of uncertainty hangs over the fate of more than 200 million women who become pregnant each year."

"The 2011 UN high-level meeting on non-communicable diseases (NCDs) called for multisectoral action including with the private sector and industry. However, through the sale and promotion of tobacco, alcohol, and ultra-processed food and drink (unhealthy commodities), transnational corporations are major drivers of global epidemics of NCDs. What role then should these industries have in NCD prevention and control? We emphasise the rise in sales of these unhealthy commodities in low-income and middle-income countries, and consider the common strategies that the transnational corporations use to undermine NCD prevention and control. We assess the effectiveness of self-regulation, public—private partnerships, and public regulation models of interaction with these industries and conclude that unhealthy commodity industries should have no role in the formation of national or international NCD policy. Despite the common reliance on industry self-regulation and public—private partnerships, there is no evidence of their effectiveness or safety. Public regulation and market intervention are the only evidence-based mechanisms to prevent harm caused by the unhealthy commodity industries."

One of the major challenges is to define the way in which participation improves health outcomes in order to replicate successful experiences. The goal of this symposium is to introduce a tool that has been used to examine participation as a process rather than an intervention: "the spidergram". The symposium speakers will present case studies from Mali, Burundi, Sri Lanka and Switzerland. Im Caritas Baby Hospital in Bethlehem werden jährlich ca. 35‘000 Kinder und Babys behandelt. Doch die meisten Partienten müssen auch nach dem Spitalaufenthalt weiter gepflegt werden. Um das oft überlebensnotwendige Wissen in Kinderpflege, Hygiene und gesunder Ernährung zu vermitteln, initiierte die Kinderhilfe Bethlehem die Mütterschule, die entscheidende Erfolge verzeichnen kann.

Alimata lebt in Koulbacou, einem kleinen Dorf in Burkina Faso. Das Gesundheitszentrum ist bloss 20km vom Dorf entfernt, aber der Weg dorthin ist sehr schlecht und während einiger Monate im Jahr völlig unbenutzbar, vor allem während der Regenzeit. Bei einer der medizinischen Kontrolluntersuche im Gesundheitszentrum hörte sie von einem speziellen Wohnhaus für schwangere Frauen, das gerade eröffnet wurde.

Der Jahresbericht 2012 der Novartis Foundation ist entsprechend der drei Arbeitsgebiete unterteilt: Gesundheitsprojekte, Think-Tank-Aktivitäten sowie Dialog und Networking, wobei der Schwerpunkt in diesem Jahr auf der psychosozialen Unterstützung bedürftiger Kinder und Waisen, insbesondere REPSSI, liegt. REPSSI - die Regional Psychosocial Support Initiative - ist eine von der Novartis Stiftung mitbegründete und unterstützte Organisation, die seit zehn Jahren hochmoderne Instrumente für die psychosoziale Unterstützung von Kindern in den Gebieten südlich der Sahara entwickelt, um ihnen zu helfen, Verluste zu bewältigen und wieder Vertrauen zu fassen.

Ursprünglich als "Förderverein für Otorhinolaryngologie der ORL-BAL" gegründet, wurde der Name dieser Tochterorganisation des Verbundes im Jahre 2011 in "ORL-BAL International" (OBI) geändert. OBI ist jener Teil der ORL-BAL, welcher Wissensaustausch betreibt und Projekte gemeinsam mit Kliniken und Fachorganisationen in Ländern mit begrenzten medizinischen Ressourcen durchführt. Die Organisation hat ihren Jahresbericht 2012 publiziert.

As a response to the shortage of doctors in rural areas of Zambia, SolidarMed since 2009 supports the Zambian National training program for Associate-Clinicians, in Zambia called "Medical Licentiates (MLs)". Chainama Colleg in Lusaka is SolidarMed's main partner within this program. The college offers post-graduate training for clinical officers to reach the bachelor degree of Medical Licentiates. Training cycles are divided into theoretical parts taught at Chainama college, and into clinical attachments (e.g. medicine, surgery, and obstetrics) at accredited District training sites, mostly rural District Hospitals. To both strengthen the newly accredited rural training site of Kafue District Hospital and support theoretical internal medicine teaching at Chainama College, SolidarMed offers an exciting opportunity for a medical practitioner with focus on general and internal medicine (as opposed to surgery and obstetrics) and an interest in capacity-building and teaching.
