MMS Nachrichten vom 03.04.2013

Es zogen bereits verschiedene Gedankenblasen am Horizont der Post-MDG-Ära vorüber: Vom globalen Gesundheitsrahmenabkommen über das Universal Health Coverage-Konzept bis zum schlichten Fortschreiben der bisherigen Gesundheitsentwicklungsziele. Anfangs März trafen sich in der botswanischen Hauptstadt Gaborone Gesundheitsminister, DirektorInnen internationaler Organisationen und VertreterInnen der Zivilgesellschaft zu einem High-Level Dialog zur Gesundheit in der Post-2015 Entwicklungsagenda.

Der Bericht des Treffens bleibt schwammig. Ausgehend von einer „Healthy Planet, Healthy People“ Vision wird das Oberziel „Maximizing health at all stages“ postuliert, um dann die mit der Reduzierung der nicht-übertragbaren Krankheiten angereicherten, bestehenden Gesundheitsentwicklungsziele (MDG 4, 5 und 6) fortzuschreiben. Diese Ziele würden für die Zeit nach 2015 neu justiert und teilweise radikalisiert.

Interessant ist, wie das von der WHO portierte Konzept Universal Health Coverage (UHC), das sich am besten mit „allgemeine Gesundheitsversorgung“ übersetzen lässt, an dem Treffen gedreht und gewendet wurde. Sollte es nicht eher Universal Health Access heissen? Schliesslich sei Access als politisch wirkungsmächtiger Begriff in den Gesundheitsdebatten schon eingeführt. Umgekehrt widerspiegelt Coverage, dass die Inanspruchnahme der Gesundheitsdienste nicht mit finanziellen Risiken verbunden ist. Im Bericht wird denn auch von „Universal Health Coverage (and Access)“ geschrieben und damit bereits ein möglicher Ausweg aus der Debatte aufgezeichnet.

Doch neben dieser semantischen Debatte stellt sich eine grundlegendere Frage: Wie verknüpft sich UHC mit der Nach-2015-Entwicklungsagenda? Ist die Erreichung der UHC nun selbst ein Ziel oder aber einfach ein Mittel, um die anderen Ziele zu erreichen?

Diese Debatte zeigt, dass das UHC-Konzept aufgrund seiner inhaltlichen Unschärfe noch nicht in den Köpfen angekommen ist. Wenn ihre PromotorInnen damit nicht Schiffbruch erleiden möchten, sollten sie um einiges konkreter werden.

Die Mutter-Kind-Gesundheit ist einer jener Faktoren, die für die gesellschaftliche Entwicklung in Entwicklungsländern entscheidend sind. Doch jährlich sterben noch rund 360'000 Frauen während der Schwangerschaft oder der Geburt. 7,6 Millionen Kinder sterben jedes Jahr, bevor sie den 5. Geburtstag feiern können. Das MMS Bulletin schaut genauer hin: Was geschieht in der internationalen Politik, um die Situation weiter zu verbessern? Welche Strategien sind wirksam? Wie kann der Zugang für Frauen und werdende Mütter zur Gesundheitsversorgung verbessert werden?

"Das Netzwerk Medicus Mundi Schweiz stellt an die GAP eine andere Anforderung als die Koordination tendenziell antagonistischer Regierungseinheiten: Die bundesrätliche Strategie müsste das Menschenrecht auf Gesundheit an den Anfang stellen und daraus ableitend definieren, wie die konkreten Beiträge der Schweiz zu gestalten sind, um dieses Recht sicherzustellen," schreibt Martin Leschhorn Strebel im Bioethica Forumm, der Schweizer Zeitschrift für Biomedizinische Ethik. FAIRMED präsentierte Resultate von Arbeit in Kamerun

FAIRMED engagiert sich in Afrika und Asien für die Prävention und Behandlung von vernachlässigten Tropenkrankheiten (Neglected Tropical Diseases, NTD). Buruli Ulcer gehört zu den 17 NTDs, die im Rahmen der weltweiten Kampagne „Uniting to combat NTDs“ bis 2020 eingedämmt werden sollen. Vom 25. bis 27. März trafen sich am Sitz der WHO in Genf Vertreter von Regierungen, NGOs und Forschungseinrichungen um den Fortschritt in der Bekämpfung von Buruli zu diskutieren. Eine Delegation von FAIRMED präsentierte Resultate von FAIRMED’s Buruli-Aktivitäten in Kamerun. Buruli ist eine mykobakterielle Hauterkrankung, die unbehandelt zu Geschwürbildung und Behinderung führt. Jährlich erkranken rund 6000 Menschen an Buruli, hauptsächlich in Afrika. Wegen ungenügender Früherkennung und Behandlung erleiden viele Buruli-Infizierte – ähnlich der Leprakranken – bleibende Behinderungen, vor allem an den Beinen. FAIRMED unterstützt das nationale Buruli-Programm sowie vier Diagnose- und Behandlungszentren in Kamerun.

FAIRMED betroffen von Unruhen in der Zentralafrikanischen Republik (ZAR): Im Zuge des Staatsstreichs in der ZAR war das Büro von FAIRMED in der Hauptstadt Bangui von den gewaltsamen Auseinandersetzungen betroffen. Bei einem Überfall wurde ein Projektfahrzeug von FAIRMED entwendet. FAIRMED schloss das Büro in Bangui vorübergehend, konnte die Projektaktivitäten im Süden des Landes aber fortführen. ZAR ist eines der ärmsten Länder Afrikas. FAIRMED unterstützt insbesondere die indigenen Minderheiten der Aka-Pygmäen im tropischen Regenwald der ZAR. Im laufenden Jahr pilotiert FAIRMED ein Gutschein-System für schwangere Aka-Frauen. Ziel des Projekts ist es, den Aka-Frauen den Zugang zu den Gesundheitseinrichtungen zu ermöglichen und damit die Mütter- und Säuglingssterblichkeit zu vermindern.

Wie jeden Tag sterben auch am Muttertag 2013 weltweit 1000 Frauen an Komplikationen während Schwangerschaft oder Geburt. «Das darf nicht sein», sagt die Bethlehem Mission Immensee und organisiert am Samstag, 11. Mai, die erste Schweizer Mutternacht. (pdf)

Tuberkulose ist in der Schweiz mit jährlich ca. 500 Fällen zwar selten, weltweit aber die zweithäufigste tödliche Infektionskrankheit. Am 22. internationalen Tuberkulose-Symposium der Lungenliga präsentierten Expertinnen und Experten neueste Forschungsergebnisse. Insbesondere bei der Entwicklung neuer Medikamente und in der Diagnostik wurden bedeutende Fortschritte erzielt.

Zugang zu medizinischer Hilfe erschwert

Die elf Monate alte Esther ist das erste Kind aus Gaza seit 10 Jahren, das im Caritas Baby Hospital behandelt wird. Die Abriegelung des Gaza-Streifens mit strengen Kontrollen und die eingeschränkte Bewegungsfreiheit innerhalb des Westjordanlandes haben dazu geführt, dass sich viele Jahre kein Kind aus Gaza auf den Weg nach Bethlehem machen konnte. Mit Hilfe einer internationalen Hilfsorganisation ist es Esther und ihrer Mutter nun gelungen, Hilfe bei den Ärzten im Caritas Baby Hospital zu erhalten.

Mutternacht - Aktionstag gegen weltweite Müttersterblichkeit

Wenn Mütter sterben, wird es Nacht. Jährlich sterben weltweit mehr als Dreihundertfünfzigtausend Mütter infolge von Komplikationen bei Schwangerschaft oder Geburt - fast ausschliesslich in Entwicklungsländern. Um diesen vermeidbaren Missstand in das Bewusstsein der Menschen zu bringen, findet in Luzern am 11. Mai 2013 unter der Projektleitung des RomeroHauses die erste Mutternacht statt. Am Samstag vor dem Muttertag rückt sie die Müttersterblichkeit in den Fokus des öffentlichen Interesses - und plädiert dafür, weltweit die Gesundheit der Mütter zu verbessern und die Armut zu verringern. Zeitgleich findet die «Mutternacht» auch in Deutschland und Österreich, Malta, Ungarn und in den Niederlanden statt.

The final report of the Consultative Expert Working Group (CEWG) on Research and Development (R&D) was presented in April 2012 and discussed at the World Health Assembly (WHA) 2012. In January 2013, the World Health Organization (WHO) Executive Board debated a draft resolution that includes, among other items, the proposal to establish a global health R&D observatory, to facilitate the implementation of health R&D demonstration projects, to review the existing coordination function for R&D, and to develop new proposals for effective mechanisms for coordination, including pooling resources and voluntary contributions. This will be discussed at the WHA in May 2013. The purpose of the proposed seminar and workshop is to examine how to secure collective action to provide global public goods through R&D and innovation for the specific health needs of low- and middle-income countries (LMICs).

"The High Level Dialogue was the culmination of six months of consultation on how health should be addressed in the post-2015 development agenda. There was broad consensus on the strengths and shortcomings of the health MDGs: the goals have raised the profile of global health to the highest political level, increased development assistance for health, and contributed to considerable improvement of health in low- and middle-income countries. The lack of attention to equity is widely regarded as one of the most significant shortcomings of the health MDGs that needs to be addressed in the post-2015 agenda. Looking forward, the post-2015 agenda needs to be universal and relevant to all countries."
