Mise en opposition malvenue entre programmes antisida et systèmes de santé

Compte tenu de la mauvaise situation économique mondiale, faut-il faire des économies dans la coopération internationale en matière de santé? C’est en tout cas ce que pense le Conseil fédéral – gouvernement de l’un des pays les plus riches de la planète. Désobéis-sant au Parlement, le Conseil fédéral refuse de présenter un message additionnel portant le budget de la coopération au développement à 0,5 pour cent du revenu national brut. Le Conseil des Etats vient d’ailleurs à juste titre de le réprimander à ce propos.

Dans le cadre des mesures générales d’économie, les critiques à l’égard des initiatives ciblant spécifiquement le sida se multiplient sur la scène internationale, et des voix s’élèvent pour demander que l’on réduise la dotation financière des programmes antisida au profit d’autres programmes de santé prioritaires, parmi lesquels – exemples volontiers mis en avant – ceux en faveur de la santé mère-enfant et ceux visant à renforcer les sys-tèmes de santé.

Or, mettre en opposition le renforcement des systèmes de santé et le renforcement des programmes antisida est un faux débat. Des études montrent que l’accès amélioré aux traitements antisida, en plus de sauver la vie de personnes atteintes du VIH/sida, a dimi-nué globalement la mortalité dans certains pays à forte prévalence du VIH. Une étude de Médecins Sans Frontières (MSF) menée au Malawi et en Afrique du Sud révèle que les taux de mortalité globale ont baissé significativement dans les régions où la thérapie an-tirétrovirale est répandue. Les effets positifs des traitements antisida sur d’autres objectifs de santé importants, notamment en ce qui concerne la santé mère-enfant, sont d’autant plus marqués dans les pays à forte prévalence du VIH.

aidsfocus.ch, la plate-forme «VIH, sida et coopération internationale» du Réseau Medicus Mundi Suisse, abordera ces questions lors d’un symposium qui se déroulera le 14 avril 2010 à Berne, auquel vous êtes cordialement invités. Des spécialistes de renom débattront de cette thématique sous différentes perspectives et susciteront la réflexion.

Comment le Fonds mondial de lutte contre le sida, la tuberculose et le paludisme fait-il face à cette évolution? Le Dr Christoph Benn, directeur Partenariat et Communication au Fonds mondial, nous le dira. La directrice de l’International AIDS Society, Robin Gorna, parlera de la situation actuelle comme d’un état d’urgence, car les difficultés d’accès aux thérapies antisida ou l’interruption des traitements accroissent le risque de formation de résistances et compromettent les acquis de ces dix dernières années. Le Dr Lydia Mungherera, femme médecin ougandaise elle-même séropositive et experte très engagée sur les questions liées au sida, nous informera sur les répercussions du définancement des programmes de lutte contre le VIH/sida dans les villages et communautés.

Comment les œuvres d’entraide et les organisations de coopération internationale en matière de santé traitent-elles ces questions? Comment utiliser aussi efficacement que possible les ressources financières de plus en plus limitées dont nous disposons? Com-ment la Suisse, en particulier la DDC, honore-t-elle son engagement, pris dans le cadre des Objectifs du Millénaire pour le Développement, de garantir d’ici à 2010, à toutes les personnes infectées par le VIH dans le monde, l’accès à une prise en charge médicale?

On the other hand, HIV and AIDS with a present high of 33.4 million people living with HIV remains a serious global problem, reaching out beyond the health sector – and a united concerted effort is needed more than ever. What are the implications of these changes and debates on macro level for Swiss NGOs working in the field of HIV, AIDS and international cooperation? What are the implications for prevention, for treatment programmes and/or on programmes for psychosocial support? How can the demand for universal access to comprehensive prevention, treatment, care and support by 2015 still be met?

Afrique du Sud Afrika-Komitee | Im Vorfeld der Fussball-WM in Südafrika zeigt die vom Afrika-Komitee mitgetragene KEESA (Kampagne für Entschuldung und Entschädigung im südlichen Afrika)den aktuellen Dokumentarfilm "When the Mountain Meets its Shadow", der anhand von vier ProtagonistInnen ungelöste Probleme aus der Apartheidszeit anschaulich darstellt. An der Vorführung in Basel werden die FilmemacherInnen Alexander Kleider und Daniela Michel sowie die Protagonisten Ashraf Cassiem und Mncedisi Twalo aus Kapstadt anwesend sein. Der Film wird aber auch an verschiedenen weiteren Orten in der Schweiz gezeigt.

Chili mission 21 | Die Küstengegend um die Stadt Concepción ist vom jüngsten Erdbeben in Chile am stärksten betroffen. Nach bangen Tagen herrscht nun Gewissheit: Die dort tätigen Mitarbeitenden von mission 21 sind am Leben. Doch auch die Naturkatastrophe in Chile zeigt leider: Die Ärmsten haben am meisten unter den Folgen zu leiden. mission 21 ist nicht auf sofortige Nothilfe, sondern auf Aufbauhilfe spezialisiert. Die chilenischen Partner von mission 21 sind in Sozialprojekten mitten unter den Schwerstbetroffenen in der Hauptstadt Santiago und in Concepción tätig. Jahrelange Aufbauhilfe in der Gegend um Concepción wird dringend nötig sein. mission 21 hat ein Spendenkonto „Wiederaufbau Chile“ eingerichtet.

Tanzanie Swiss TPH | The Tanzanian government in cooperation with the Tanzania Commission of AIDS (TACAIDS) is in the final stages of drafting an HIV Prevention Strategy for the Tanzania Mainland. This process was supported by the SCIH (Swiss Centre for International Health/Swiss TPH) Senior Education Advisor Heinrich Heinrichs. The HIV Prevention Strategy for Tanzania Mainland is based on a thorough review of HIV epidemiology in Tanzania (magnitude, trends and drivers) and the current HIV prevention response. The aim is to guide comprehensive multi-sectoral HIV prevention efforts with a major emphasis on full scale proven and prioritised HIV prevention interventions that address the drivers of the epidemic, while ensuring that appropriate HIV prevention efforts are pursued in disproportionately affected population groups.

Humanité – le nouveau magazine de la Croix-Rouge suisse
Promotion de la santé dans le contexte de la coopération internationale
(F) CRS | Le 15 février paraît le premier numéro du nouveau magazine de la Croix-Rouge suisse (CRS), que l’organisation humanitaire destine trimestriellement à ses donateurs. Cette première édition traite de la promotion de la santé dans le contexte de la coopération internationale, thème illustré par un reportage sur l’ébauche d’un système de soins au Soudan et le portrait de la responsable du programme de lutte contre le sida au Swaziland. D’autres récits déclinent les multiples facettes de l’engagement des collaborateurs, tant rémunérés que bénévoles, de la CRS et de ses organisations membres.

PAHO: Distinction pour le programme de santé d’Enfants du Monde
Domaine de la santé des jeunes femmes, des mères et des nouveau-nés
(F) El Salvador EdM | L'Organisation Panaméricaine de la Santé a honoré le programme de santé maternelle et néonatale appuyé par Enfants du Monde, comme un des trois meilleurs de la région des Amériques. Les partenaires de ce programme, à savoir le Ministère de la Santé salvadorien et l'organisation locale Conertación Educativa de El Salvador, ont reçu un prix doté de 5'000 dollars. Il distingue ce programme de santé pour son action dans le domaine de la santé des jeunes femmes, des mères et des nouveau-nés. Depuis trois ans, Enfants du Monde et ses partenaires améliorent les services de santé et proposent des cours en santé pour les femmes enceintes, leur famille et leur communauté. Ce programme touche 16 communes particulièrement défavorisées.

(F) DDC | Une grande majorité de la population suisse considère que l’aide au développement est une stratégie efficace pour résoudre les problèmes de pauvreté et de développement dans le monde. Les personnes interrogées préconisent en priorité le renforcement du commerce équitable et la résolution des problèmes se posant à l’échelle planétaire. Ces constats ressortent de l’enquête représentative menée auprès de la population suisse sur mandat de la Direction du développement et de la coopération (DDC) et d’Alliance Sud.

Global Fund | Virtual elimination of mother to child HIV transmission by 2015 is now within reach if current rates of progress by Global Fund-supported programs and other efforts are maintained. Malaria may be eliminated as a public health problem within a decade in most countries where it is endemic. Tuberculosis prevalence in many countries is declining and the international target of halving TB prevalence could be met by 2015.

Ehiopie WFPHA | The Journal of Public Health Policy publishes a report of the first working group of the High-Level Taskforce on Innovative International Financing for Health Systems (Taskforce). It "reveals two different approaches to health systems strengthening (HSS) in low-income countries (LICs). One approach, developed by the WHO, is a plan for more health centres, staffed with ‘classic’ health workers: physicians, nurses and midwives. The other approach, has been developed by the World Bank, UNICEF, UNFPA, and the Partnership for Maternal, Newborn and Child Health. The ‘Marginal Budgeting for Bottlenecks (MBB) approach’, as it is called, requires more health posts and health workers with more basic training."

MMI | ”The Call for Ethical Clinical Trials in Developing Countries has been formulated out of concern about the shift in many clinical drug trials away from the richer nations and towards developing countries, and the associated ethical violations. It represents a call to action for policy makers, regulators and pharmaceutical companies to protect vulnerable trial subjects. The call was drafted by a worldwide coalition of health and human rights organizations and experts led by the Wemos Foundation. One year after the launch of the FairDrugs.org campaign, in February 2009, the number of supporters of the ‘Call for Ethical Clinical Trials in Developing Countries’ is nearly 170, including leading figures in the field of medicines and ethics. They lend enormous weight to the campaign and motivate politicians, policymakers and pharmaceutical companies to pay attention to ethics.”

Februar 2010 einen Hintergrundartikel zur Rekrutierung von Gesundheitspersonal in einkommenschwachen Ländern: "In westlichen Ländern werden die Menschen immer älter. Das stellt auch deren Gesundheitssysteme vor Herausforderungen. Die Weltgesundheitsorganisation will Regeln für die Rekrutierung von Fachpersonal erstellen. Doch die Schweiz tut sich schwer damit," kritisieren NGOs wie das Netzwerk Medicus Mundi Schweiz.
