| L’IUED a organisé en mai 2003 un atelier (1) réunissant des experts d’Afrique, d’Europe et du Canada pour étudier le financement communautaire des services de santé au Mali et jeter les bases d’un travail dans la Région de Sikasso, où l’IUED travaille depuis plus de 20 ans dans le cadre d’un programme de coopération financé par la Direction du Développement et de la Coopération.

| The Global Health Programme at the Graduate Institute of International and Development Studies (IHEID) in Geneva was established after the merger of the former Institut de Hautes Etudes Internationales (HEI) and the Institut Universitaire d’Etudes du Développement (IUED). Since 2009, IHEID is replacing IUED as a member of Medicus Mundi Switzerland, the Network Health for all.

| Deux grandes approches sont généralement retenues pour définir la pauvreté. Une approche socio-économique qui définie la pauvreté comme étant une rupture du lien social du fait d’une marginalisation économique, sociale et culturelle. Une seconde approche purement économique définie la pauvreté par rapport à un seuil monétaire et qui se traduit par une insuffisance de ressources ne permettant pas la couverture de besoins jugés essentiels comme se nourrir, se loger, se soigner etc. C’est cette seconde approche qui est retenu dans le discours et les stratégies institutionnelles actuelles et c’est ce dont je vais parler.

| Par son enracinement dans la culture des Hauts-Plateaux de Madagascar, par la transparence de sa gestion et son fonctionnement démocratique, le mouvement des Mutuelles Pharmacies Communautaires participe d’un esprit de responsabilisation et d’appropriation par la communauté, d’équité, et de lutte contre la pauvreté. Aujourd’hui, les Mutuelles Pharmacies Communautaires sont menacées par la concurrence d’un système introduit par l’Etat dans l’esprit de la « mondialisation ».

| Avec un taux d’alphabétisation de 23%, un nombre moyen de 6.7 enfants par femme, une mortalité infantile de 152‰ , le Mali, classé au 171ème rang de l’Indexe du Développement Humain IDH, présente une gamme d’indicateurs de développement très inquiétante. Quant à la situation sanitaire, on note une très haute prévalence des infections respiratoires aiguës, des diarrhées et du paludisme, qui représente à lui seul 30 à 40% des motifs de consultations. Depuis quelques années, l’État malien répond à ces défis avec une politique sectorielle de santé claire et cohérente.
