The publication of the presentations made during the Spring Symposium on 21 March 2007 on “Human resources – beyond the declarations” brings a wealth of contributions. However, thinking human resources within the frame of the traditional definitions of health professions will hardly lead to comprehensive solutions. We need to question the usual boundaries between professions and create innovative definitions challenging some "sacred cows". An example is the Surgeons Technicians who for years have been responsible for most of simple abdomen procedures (including Caesarian operations) in rural Mozambique with excellent results. The Karolinska Institute in Sweden has promoted and followed up the experience. Without such innovations, the human resource shortage problem will be still burdening developing countries for centuries. An inventory of innovative experiences being tested all over the world would be a great contribution.
