Die Deklaration von Alma-Ata lässt sich auch 40 Jahre nach ihrer Verabschiedung nicht vom Tisch wischen. Was steht hinter ihrer Aktualität?
Es kommt nicht häufig vor, dass eine Konferenz und ihre Schlusserklärung auch vierzig Jahre später noch zu reden geben und auf ihre Relevanz hin abgeklopft wird. Die Alma-Ata-Deklaration zur Basisgesundheitsversorgung ist ein solches Dokument. Die Weltgesundheitsversammlung führt eine eigene Session dazu durch, die Weltgesundheitsorganisation plant eine Nachfolgekonferenz in Kasachstan, zivilgesellschaftliche Plattformen führen eigene Konferenzen durch – und auch unser Netzwerk widmet seinen diesjährigen Jahresschwerpunkt der Zukunft der Basisgesundheitsversorgung und von Gesundheit für alle.
Weshalb also die ununterbrochene Auseinandersetzung mit der Alma-Ata-Deklaration? Eines ist sicher: Ganz offenbar ist sie noch nicht umgesetzt, denn sonst würde niemand über sie sprechen wollen. Gleichzeitig müssen die Kernaussagen noch so anziehend sein, dass sie nicht einfach als kalter Kaffee daherkommen.
Ich denke, der Kern liegt im fundamental demokratischen Ansatz, der in der Deklaration postuliert wird. Die Legitimität jeder Gesundheitspolitik speist sich aus dem partizipatorischen Prozess, der dahinter steht. Es sind die umfassend verstandenen gesundheitlichen Bedürfnisse der Menschen, der Gemeinschaften und der Gesellschaften, welche sich in einer qualitativ guten Basisgesundheitsversorgung ausdrücken.
Dieser demokratische Ansatz der Alma-Ata-Deklaration steht im Widerspruch zu den unterschiedlichsten kommerziellen Interessen, die den globalen Gesundheitsmarkt prägen. Deshalb bleibt die Debatte rund um die Basisgesundheitsversorgung und Gesundheit für alle auch heute noch umstritten und höchst aktuell.

Internationale zivilgesellschaftliche Organisationen verabschieden ein Statement
Im Vorfeld der 71. Weltgesundheitsversammlung (WHA), organisierte der Genfer Global Health Hub (G2H2) einen ausgezeichneten Workshop zum Thema "40 Jahre Alma-Ata: Wie kann Gesundheit für alle in die Gegenwart und Zukunft übersetzt werden?" Akademiker und zivilgesellschaftliche Organisationen aus der ganzen Welt debattierten lebhaft und hoffen, dass die im Workshop diskutierten Ideen in naher Zukunft realisiert und die Woge der Inspiration auch für viele TeilnehmerInnen der WHA in den kommenden Jahren aufrecht erhalten bleiben könne. "More than a decade after the adoption of the WHO Framework Convention on Tobacco Control, there is compelling evidence that raising tobacco prices substantially through taxation is the single most effective way to reduce tobacco use and save lives. Similarly, alcohol taxation is a cost-effective way to reduce alcohol consumption and harm. With growing evidence, sugar taxes are another fiscal tool to promote health and nutrition. Eine Analyse des ersten Jahres des neuen WHO Direktors Dr. Tedros
Dr. Tedros wurde als erster Afrikaner vor einem Jahr zum neuen Direktor der Weltgesundheitsorganisation (WHO) gewählt. Damals nannte er vier Schwerpunkte, die er in seiner Politik prioritär verbessern wollte: Einführung der universellen Krankenversicherung (Universal Health Coverage), Gesundheit in Krisensituationen, Gesundheit von Müttern, Kindern und Jugendlichen sowie die gesundheitlichen Auswirkungen von Klima- und Umweltveränderungen.

Die Schweiz unterstützt das Forschungsprogramm und sitzt im Vorstand
Mitte März fand in Genf die 31. Vorstandssitzung des Human Reproduction Programme (HRP) statt. Das HRP-Mandat besteht in der Förderung, Koordination, Unterstützung, Durchführung und Auswertung von Forschung zu sexueller und reproduktiver Gesundheit und Rechten, insbesondere in Entwicklungsländern. Das Programm wird von der WHO ausgerichtet und durchgeführt und von UNFPA, UNICEF, Weltbank, UNDP und WHO mitfinanziert. Zahlreiche Beobachter, darunter Medicus Mundi Schweiz, nahmen an der Verwaltungsratssitzung teil. Die Schweiz unterstützt über die DEZA das HR-Programm mit CHF 1.25 Mio. pro Jahr und hat einen Sitz im Vorstand. Commission calls for urgent action against chronic diseases
"Despite the many proven interventions and commitments to combat NCDs, progress has been slow and uneven globally. The WHO Independent High-level Commission on NCDs was convened by the WHO Director-General in October 2017 to advise him on how countries can accelerate progress. The Commission’s report includes six key recommendations."

Sexuelle Gesundheit Schweiz Wahrnehmung aus Sicht der Schule, der Eltern und Jugendlicher und mögliche Perspektiven für eine umfassende Sexualaufklärung in der Schweiz. Die Menschenrechte, welche sich auf die Sexualität beziehen, gelten für Kinder wie für Erwachsene und sollten Teil einer nachhaltigen Bildung (LP 21) sein. Sie sind Gegenstand des Forschungsprojekts «die sexuellen Rechte als Grundlage der Sexualaufklärung in der Schweiz», das sich in der zweiten Etappe mit der Sicht des professionellen Akteursystems der öffentlichen Schule auseinandersetzt. An dieser Tagung werden die Ergebnisse aus Umfragen mit Lehrpersonen, Fachpersonen der sexuellen Gesundheit und weiteren Involvierten im Bereich der Schulgesundheit präsentiert. Es wird aufgezeigt welche Vorstellungen die verschiedenen Akteursysteme von Sexualaufklärung basierend auf den sexuellen Rechten haben und inwiefern sie den Unterricht danach gestalten. ECTE The European Course in Tropical Epidemiology ECTE is 3-week intensive course on basic epidemiology and biostatistics. The course is suitable for a wide range of health professionals, including medics, nurses, biologists and anthropologists working in the health sector. The course is intended for staff working in low and middle income countries as health programme managers or with an interest in public health in tropical settings. Swiss TPH The course introduces the basic principles of strategic programme and project design and management, taking into consideration local, district, national and international/global evidence and realities and health system strengthening needs. It focuses on priority setting and practical aspects of the planning and implementation process, through the use of a simulation exercise and computer based support. Designed as a refresher course for professionals working in the field of programme and project planning, design and evaluation, the course is also open to participants with a DAS Health Care and Management in Tropical Countries (or equivalent) and is an optional module within the joint Master of Advanced Study in International Health.
Swiss TPH Health in detention is a one-week course offered by Swiss Tropical and Public Health Institute (Swiss TPH), an associated institute of the University of Basel, in collaboration with the International Committee of the Red Cross (ICRC). This course is open to participants working in the field of health in detention, nationally or internationally, and uses a multi-disciplinary approach to bring together academia, clinical, public health and legal experts.

René Stäheli löst Thomas Vogel ab
Das Netzwerk Medicus Mundi Schweiz hat an seiner Mitgliederversammlung vom 30. Mai 2018 den Vorstand und sein Präsidium neu gewählt. Nach zehn Jahren ist Thomas Vogel als Präsident zurückgetreten. Er bleibt aber weiterhin im Vorstand tätig. Weiter sind Maya Natarajan, Carlo Santarelli und Vreni Wenger nicht mehr zur Wiederwahl angetreten. Als neuen Präsidenten haben die Mitglieder René Stäheli, Geschäftsführer von Fairmed, bestimmt. FAIRMED trifft Massnahmen in der Zentralafrikanischen Republik
Der Ausbruch von Ebola im Nordwesten der Demokratischen Republik Kongo droht, auf die angrenzenden Staaten überzuschwappen – darunter auch die FAIRMED-Projektgebiete im Süden der Zentralafrikanischen Republik. In Anbetracht des hohen Risikos trifft FAIRMED derzeit erste Vorkehrungen, um im Falle einer Epidemie in der Zentralafrikanischen Republik vorbereitet zu sein. Auf Anfrage der französischen Nachrichtenagentur afp gab FAIRMED-Programmverantwortlicher Dr. med. Bart Vander Plaetse ein Statement zum derzeitigen Ebola-Ausbruch ab. Daraufhin griffen zahlreiche internationale Medien wie die Japan Times, Radio-Canada und die brasilianische Zeitung Folha de Goias den Artikel in ihrer digitalen Berichterstattung auf. SolidarMed is now tackling malnutrition in two countries. It is estimated that 45% of deaths among children are linked to malnutrition. Malnutrition during childhood has a lasting effect on children’s chance of survival and development. With funding from SwissSolidarity, SolidarMed started the project “Children’s Health First” in Lesotho. In Mozambique, a “Lean Season Nutrition Response and Resilience Building” - initiative funded by the World Food Programme and UKAID started in March.
The SESOTHO Trial aims to answer this question by comparing WHO guidelines for switching patients with alternate guidelines of lower thresholds, to improve outcomes and decrease the risk of resistant mutations. The trial is set up by the consortium composed of SolidarMed, SwissTPH, University Hospital Basel, Ministry of Health of Lesotho and the Department of Biomedicine, University of Basel, who’s main aim is to support the Lesotho government to reach the 90-90-90 UNAIDS Targets by 2020. Kampagne für bezahlbare Medikamente - Medienmitteilung
"Zugangsbeschränkungen für lebensrettende Medikamente sind längst kein Problem mehr, das nur Entwicklungs- und Schwellenländer betrifft. Sogar das Schweizer Gesundheitssystem ist kaum mehr in der Lage, für die überrissenen Preise insbesondere von neuen Krebstherapien aufzukommen. Der Bundesrat muss deshalb handeln.

Das Gesundheitssystem in Gaza ist durch die Zahl der Verletzten überlastet und aufgrund der Blockade zudem in seinem Handlungsrahmen eingeschränkt. Hunderte von Verletzten, die dringend Reha-Massnahmen benötigen, können nicht versorgt werden. Die humanitäre Hilfsorganisation Handicap International (HI) weitet ihre Aktivitäten in Gaza aus, um Notfalltherapien für die Verletzten bereitzustellen. Derartige Sofortmassnahmen erhöhen die Heilungschancen beträchtlich.
