Von diesen nicht-übertragbaren Krankheiten sind Schwellen- und Entwicklungsländer unterdessen massiv betroffen: 80% der Todesfälle, die auf Herz-Kreislauf-Erkrankungen, Krebs oder Diabetes zurückzuführen sind, ereignen sich unter den armen und unterprivilegierten Bevölkerungsschichten der Schwellen- und Entwicklungsländer.

World Health Organization says lower-income countries will record the biggest proportional increases

The Guardian "Global cancer cases are predicted to rise by more than 75% by 2050, according to the World Health Organization. Latest figures from the WHO’s cancer arm, the International Agency for Research on Cancer, makes plain the growing burden of cancer, rising from 14.1 million new cases and 8.2 million deaths worldwide in 2012 to 20 million new cases and 9.7 million deaths a decade later. The IARC predicts there will be more than 35 million new cancer cases by 2050, an increase of 77% from 2022 levels, and that deaths will have nearly doubled since 2012 to more than 18 million."

Fehlende Medikamente begünstigen die hohe Zahl an Krebstodesfällen in Afrika

Eine aktuelle Studie aus Botswana belegt, was schon seit langem bekannt ist: In Afrika fehlen Medikamente und Therapiemöglichkeiten zur Behandlung der mittlerweile grössten Todesursache des Kontinents. Die Sterblichkeitsraten im südlichen Afrika gehören laut der Studie zu den höchsten der Welt, was auch auf die späte Diagnostik und das Fehlen von Screening-Programmen zurückzuführen ist. ​Bis 2030 rechnet man mit etwa 1 Million Krebstodesfällen jährlich. In Botwana liegt die Krebsstreblichkeitsrate schon heute bei über 63%. Zu den häufigsten Krebsarten zählen Gebärmutterhals-, Brust-, Prostata- und Lungenkrebs. "Krebs bedroht die Bevölkerung in Afrika südlich der Sahara in einem Ausmaß, das schnelles Handeln erfordert", mahnt der Autor der Studie, Richard Marlink. Die Studie bietet einen Leitfaden, wie der Zugang zu hochwirksamer Behandlung, wie sie in anderen Regionen der Welt zur Verfügung steht, verbessert werden kann.

Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050."

The Lancet "It is time to end all forms of stigma and discrimination against people with mental health conditions, for whom there is double jeopardy: the impact of the primary condition and the severe consequences of stigma. Indeed, many people describe stigma as being worse than the condition itself. This Lancet Commission report is the result of a collaboration of more than 50 people worldwide. It brings together evidence and experience of the impact of stigma and discrimination and successful interventions for stigma reduction. We include material that brings alive the voices of people with lived experience of mental health conditions (PWLE). This is right in principle because we agree with the view of nothing about us without us. It is right in practice because the evidence summarised in this report shows that PWLE are the key change agents for stigma reduction. For these reasons, this report has been co-produced by people who have such lived experience and others who do not."

Bericht über das erste hochrangige Treffen der Staats- und Regierungschefs zu NCDs bei der Generalversammlung der Vereinten Nationen (UNGA)

Health Policy Watch In den meisten Teilen Afrikas werden NCDs nur in Gesundheitseinrichtungen in Großstädten behandelt, so dass chronische Krankheiten meist nicht diagnostiziert werden. Die Mehrheit der Afrikaner mit Bluthochdruck weiß nichts von ihrer Erkrankung. Dies ist nur eine der nüchternen Statistiken, die in dem bisher umfangreichsten Datenvisualisierungsinstrument der Weltgesundheitsorganisation (WHO) enthalten ist, welches die Länder dabei unterstützen soll, das Ausmaß und die Kosten der globalen NCD-Krise zu erkennen. Die Einführung des Instruments, wurde von einem bahnbrechenden Bericht begleitet und erstmals an der Klausurtagung der im April gegründeten Präsidialgruppe zu NCDs am 21. September 2022, während der UNGA-Versammlung in New York vorgestellt.

"Around a quarter of the population in Switzerland suffers from chronic diseases. These are some of the most frequent causes of death and cause physical and mental stress as well as high health care costs. Swiss TPH has extensive expertise in long-term studies involving biobanks that conduct research into chronic diseases and the impact of behavioural and environmental factors. Samples stored in biobanks and linked to the health data of the sample donors supply the evidence for successful prevention and health promotion. They also help us to understand the long-term health impacts of air pollution, heatwaves, traffic noise or chemicals. Swiss TPH is therefore making an important contribution to health monitoring in collaboration with other research institutions. In addition, biobanks are also the bedrock of medical progress in the early detection and diagnosis of chronic diseases, especially in cooperation with private companies."

"In sub-Saharan Africa (SSA), urgent action is needed to curb a growing crisis in cancer incidence and mortality. Without rapid interventions, data estimates show a major increase in cancer mortality from 520 348 in 2020 to about 1 million deaths per year by 2030. Here, we detail the state of cancer in SSA, recommend key actions on the basis of analysis, and highlight case studies and successful models that can be emulated, adapted, or improved across the region to reduce the growing cancer crises. Recommended actions begin with the need to develop or update national cancer control plans in each country. Plans must include childhood cancer plans, managing comorbidities such as HIV and malnutrition, a reliable and predictable supply of medication, and the provision of psychosocial, supportive, and palliative care."

"The Lancet Oncology Commission on cancer in sub-Saharan Africa highlights the rapidly escalating cancer burden in this resource-poor area of the world. Long affected by infectious diseases and malnutrition, the population of this region is now increasingly afflicted by non-communicable diseases, including cancer. However, the fragile health systems in the 46 sub-Saharan African nations are poorly equipped to tackle this looming health crisis, with many problems including incomplete cancer data registries, poor availability of and access to screening, diagnostics, and treatment, insufficient provision of palliative care, and a huge shortfall of trained medical personnel compounding the deteriorating situation. In this Commission, the authors describe these challenges in detail, and propose actions that must be taken urgently to address the escalating catastrophe. Ultimately, political will and effective collaboration between multiple stakeholders is needed to reach the achievable goal of improved cancer care in sub-Saharan Africa."

"Non-governmental organizations play a vital part in the achievement of the Sustainable Development Goals as defined by the United Nations. These Goals also include targets related to noncommunicable diseases. However, non-governmental organizations have played a limited role in this area despite such diseases causing the bulk of morbidity and mortality worldwide. Through their activities, non-governmental organizations should aim to strengthen health systems, yet they often only support these for a single disease. Mali, like many other low- and middle-income countries, is facing an increasing burden of diabetes and a health system not adapted to address this challenge. Santé Diabète, a non-governmental organization based in Mali since 2003, has been working specifically on diabetes, and has developed a wide range of activities to improve the national health system."

This is according to Katie Dain, CEO of the NCD Alliance, who urged attendees at a high-level NCD briefing before the WHA on 22 May, to highlight solutions in order to encourage countries that it is possible to address NCDs. Medikamente mit Patentschutz sind in Ländern mit niedrigem und mittlerem Einkommen für viele Patienten oft unerschwinglich, berichtet die Weltgesundheitsorganisation (WHO). Dazu gehört Insulin. Obwohl sich die Prävalenz von Diabetes in diesen Ländern in den letzten 30 Jahren fast verdoppelt hat, ist eine Behandlung mit Insulin zu teuer. Drei Hersteller kontrollieren praktisch den gesamten Markt und fehlender Wettbewerb hält die Preise hoch. Die WHO empfiehlt nun Insulinanaloga auf ihrer Liste. Gleichzeitig plädiert die WHO erstmals dafür, eine Arbeitsgruppe einzurichten, die sich mit den hohen Preisen von Medikamenten befasst, die als unentbehrlich gelten, aber aufgrund ihres Hochpreissegments nicht empfohlen werden können. Dazu gehören beispielsweise auch eine Vielzahl von wichtigen Krebsmedikamenten. Mehrere Regierungen mit knappen Kassen haben bereits versucht, Patente zu umgehen, aber solche Schritte wurden regelmäßig - und lautstark - von der Pharmaindustrie abgelehnt.

Die langfristigen Folgen von E-Zigaretten sind noch zu wenig erforscht, aber es zeichnet sich ab, dass diese Produkte nicht harmlos sind, warnte die Weltgesundheitsorganisation in ihrem kürzlich veröffentlichten Bericht. Der globale Umsatz ist in den letzten Jahren um mehr als das Doppelte gestiegen. Besorgniserregend sei vor allem die zunehmende Nutzung durch Jugendliche, die gezielt durch eine Vielzahl an unterschiedlichen Geschmacksstoffen gelockt werden. Bessere Regulierungen sind notwendig.

Medicus Mundi Schweiz Hundert Jahre nach der Entdeckung des Insulins ist dieses für die Bevölkerung von einkommensschwachen Ländern nach wie vor schwer erhältlich. Der ungleiche Zugang zu dieser grundlegenden medikamentösen Behandlung, lässt sich durch die Produktionskosten allein nicht erklären. Die Eigenheiten des Insulin-«Marktes» erlauben den Hersteller- und Verteiler*innen, ihre Bedingungen und oft auch den Preis zu diktieren. Wie bei anderen Produkten der Pharmaindustrie verschaffen die «Unzulänglichkeiten des Marktes» den Anbieter*innen einen unfairen Vorteil, während die Patient*innen in jeder Hinsicht den Preis dafür bezahlen. Wenn es darum gehen soll, eine wirksame Behandlung zu den niedrigsten Kosten anzubieten, müssen die «Unzulänglichkeiten des Markes» von den zuständigen Behörden korrigiert werden – vielleicht unter dem Druck jener, die gemeinsam der Ansicht sind, dass der Zugang zur Gesundheit kein Privileg sein darf. Despite South Africa’s Sugar Tax Success, Food Industry Impedes Similar Taxes in Neighbouring Countries

"CAPE TOWN – South Africans are consuming fewer sugary drinks since their government introduced a tax on these in 2018 – but powerful multinational food and beverage companies are inhibiting neighbouring countries from following suit, according to research published on Tuesday. South Africa was the first African country to impose a tax of approximately 10% on the sugar in sweetened soft drinks in an attempt to curb rising obesity and related diseases including diabetes and hypertension. Last week, the Lancet published a four-year study that looked at the sugar consumption of 113,000 South African households and found per capita daily sugar consumption dropped from 16.25g before the tax was announced to 10.63g in the year after the tax was implemented."

Fragwürdiger Governanz-Prozess zur Bekämpfung von NDCs

Trotz einer Reihe von hochrangingen Treffen der Vereinten Nationen (UN) und im Anschluss verabschiedeten politischen Erklärungen ist die globale Bekämpfung von Nicht-übertragbaren Krankheiten (NCDs) bislang unzureichend. Die vorliegende Analyse untersucht mögliche Ursachen für den langsamen Fortschritt, indem sie darlegt, wie der globale politische Prozess durch die Konsultation mehrerer unterschiedlicher Interessengruppen beeinflusst wird. Vor allem der Einfluss des Privatsektors und nicht-staatlicher Akteure einschliesslich der Alkohol- und Getränkeindustrie wirft Fragen auf: Welche Interessenskonflikte entstehen, wenn gesundheitsschädliche Industrien an gesundheitspolitischen Entscheidungen beteiligt sind? Inwieweit besteht die Notwendigkeit, die Einschluss-/Ausschlusskriterien in Konsultationsprozessen für globale Politikgestaltung und Governance zu NCDs zu überdenken?

The Lancet Die Politik unternimmt zuwenig um das globale Problem von übergewichtigen und unterernährten Menschen in den Griff zu bekommen. Obwohl schon seit Jahrzehnten bekannt, ist die Politik nicht in der Lage den dringensten gesundheitlichen Problemen der Fehlernährung entgegenzusteuern. Wirtschaftliche Interessen und zuwenig Druck von der Öffentlichkeit sind nach Ansicht der Lancet-Kommission verantwortlich für die "policy inertia". Die Lancet-Kommission warnt in ihrem Bericht vor einer globalen Syndemie, die aus dem Ruder läuft: Die drei Pandemien Übergewicht, Unterernährung und Klimawandel bedingen und verstärken sich gegenseitig. The Guardian China hat ein ernstzunehmendes Problem mit der zunehmenden Fettleibigkeit seiner Bevölkerung: 42,3% der chinesischen Erwachsenen waren 2011 übergewichtig oder fettleibig, verglichen mit 20,5% im Jahr 1991. Coca-Cola ist mitverantwortlich an dieser Misere, denn anstatt die ChinesInnen zu Diät und zu einer verminderten Zuckeraufnahme aufzufordern, rät das von Coca-Cola mitfinanzierte "International Life Science Institute (ILSI)", lediglich sich mehr zu bewegen und mehr Sport zu treiben. Kein Wunder, denn China ist einer der grössten Abatzmärkte von Coca-Cola. Ein kürzlich erschienener Bericht im British Medical Journal legt die Machenschaften des Unternehmens offen. Neuer Lancet Bericht zur globalen psychischen Gesundheit

The Lancet Bereits vor einem Jahrzehnt hatte das Lancet mit einer Reihe von Publikationen die Politik aufgefordert, mehr in Diagnose und Behandlung von psychischen Erkrankungen zu investieren. 10 Jahre später leidet bereits über eine Millarde Menschen an mentalen Problemen und wenig hat sich geändert. Eine neu beauftragte Lancet Kommission beschäftigt sich nun unter verändertem Blickwinkel mit der Problematik: Im Rahmen der SDGs (UN Nachhaltigkeitsziele) geht es nicht mehr nur um Schadensbegrenzung, sondern um eine umfängliche Verbesserung der psychischen Gesundheit aller Menschen weltweit. Die Lancet Arbeitsgruppe zeigt auf, wie Armut und finanzielle und soziale Ungleichheit die Enstehung nichtübertragbarer Krankheiten förderen und umgekehrt nichtübertragbare Krankheiten Menschen in den Ruin treiben und die wirtschaftliche Entwicklung ganzer Staaten beeinträchtigt. Notwendige Fortschritte in neun der siebzehn UN-Nachhaltigkeitszielen könnte den vorzeigen Tod durch NCDs bis 2030 um ein Drittel senken, so die Arbeitsgruppe. Medicus Mundi Schweiz Diabetes–Epidemie in Mexiko, Herz-Kreislauferkrankungen in Tanzania – längst sind nicht-übertragbare Krankheiten in einkommensschwachen Ländern angekommen und stellen die dortige Gesundheitsversorgung vor grosse Herausforderungen. Die Politik hat das Problem lange Zeit verkannt und damit der Ausbreitung der globalen Nahrungsmittel- und Tabakindustrie Tür und Tor geöffnet. Deren gesundheitsschädigende Produkte sind eine wichtige Ursache für die Verbreitung der nichtübertragbaren Krankheiten in Entwicklungs- und Schwellenländernhaben die Epidemie in den Entwicklungsländern geschürt. Wie kann die Politik dieser Entwicklung noch entgegensteuern, welche Rolle übernehmen zivile Organisationen und welche Verantwortung trägt die Schweiz in der Bekämpfung von chronischen Krankheiten? Im bevorstehenden "UN-High-level meeting" zu nicht-übertragbaren Krankheiten ist die internationale Staatengemeinschaft gefordert, endlich die notwendigen politischen Weichen zu stellen. MMS Vernehmlassungsantwort zum Vorentwurf des Tabakproduktegesetzes

Medicus Mundi Schweiz Der Bundesrat hat ein Vernehmlasssungsverfahren zum zweiten Vorentwurf zum Bundesgesetz über Tabakprodukte eröffnet. Das Netzwerk Medicus Mundi Schweiz (MMS) hat den Entwurf analysiert und empfiehlt eine tiefgreifende Überarbeitung. Es äussert schwere Bedenken, was die derzeitige Ausgestaltung des Gesetzes anbelangt. Der Vorentwurf lässt weiterhin den Export von hoch gesundheitsschädlichen Tabakprodukten zu, die in der Schweiz zum Verkauf nicht zugelassen sind. Damit steht der Vorentwurf in Widerspruch zu den Zielen der internationalen Zusammenarbeit der Schweiz wie auch in Widerspruch zur Schweizer Gesundheitsaussenpolitik (GAP). Das Gesetz muss die Gesundheit der Menschen in der Schweiz und weltweit, insbesondere in Entwicklungs- und Schwellenländern, schützen. MMS erwartet, dass es die Schweizer Bemühungen in der globalen Gesundheit nicht untergräbt und den Ruf der Schweiz als Sitzstaat verschiedener nationaler und internationaler Gesundheitsorganisationen nicht gefährdet. The Lancet Die globale Umweltverschmutzung hat in den letzten Jahrzehnten extrem zugenommen und bedroht überdurchschnittlich vulnerable Bevölkerungsgruppen in Ländern mit mittlerem und schwachem Einkommen. 92% der Todesfälle aufgrund von Umweltverschmutzung ereignen sich in diesen Ländern, 70% davon aufgrund chronischer Erkrankungen. Die neu gegründete "Lancet Kommission zu Umweltverschmutzung und Gesundheit" mahnt mit sechs Empfehlungen zu raschem Handeln und kritisiert die internationale Entwicklungs- und Gesundheitszusammenarbeit das Problem zu spät erkannt zu haben. Auch im aktuellen WHO „Global Action Plan for the Prevention and Control of Non-Communicable Diseases“, so die weitere Kritik, wird das Problem kaum erwähnt. MMS Weltweit leiden fast 450 Millionen Menschen an psychischen Störungen; mehr als 75% davon leben in Entwicklungsländern. Laut Weltgesundheitsorganisation erhalten 85% der Personen mit schweren psychischen Störungen in Entwicklungsländern überhaupt keine Behandlung. Zum ersten Mal anerkennen Entscheidungsträger weltweit die Förderung der psychischen Gesundheit und des Wohlergehens (SDG Ziel 3.4) sowie die Prävention und Behandlung von Substanzmissbrauch (SDG ZIel 3.5) im Rahmen der Nachhaltigen Entwicklungsziele (SDG) an. Sofern die Regierungen die psychische Gesundheit nicht angehen, werden sie die Ziele von SDG 3 nicht erreichen. Das Netzwerk Medicus Mundi Schweiz behandelt die Frage der psychischen Gesundheit als Teil seines Jahresthemas der "nichtübertragbaren Krankheiten". MMS Die steigende Lebenserwartung der Weltbevölkerung wird das Bild der Gesellschaften des 21. Jahrhunderts zunehmend prägen und unsere Gesellschaften stückweise verändern. Zwei Drittel der alten Menschen leben schon heute im globalen Süden und mehr als die Hälfte mit niedrigsten Einkommen und teilweise unter prekären sozialen und gesundheitlichen Bedingungen. Bislang eher ein „blinder Fleck“ in der Entwicklungszusammenarbeit sehen sich WissenschaftlerInnen und die vor Ort arbeitenden Organisationen zunehmend mit den (gesundheitlichen) Auswirkungen dieses Prozesses konfrontiert. This global status report on prevention and control of NCDs (2014), is framed around the nine voluntary global targets. The report provides data on the current situation, identifying bottlenecks as well as opportunities and priority actions for attaining the targets. The 2010 baseline estimates on NCD mortality and risk factors are provided so that countries can report on progress, starting in 2015. In addition, the report also provides the latest available estimates on NCD mortality (2012) and risk factors, 2010-2012.

Public health specialists and clinicians alike agree that Humanity faces a global pandemic of chronic diseases in the 21st century. This article discusses the implications of this pandemic on another global issue, the health workforce. Because both issues are particularly acute in Sub-Saharan Africa (SSA), it focuses on this region and uses Cameroon as a case in point. It first gauges the epidemic of chronic conditions in SSA. Then it discusses the implications of chronic conditions for the reshaping of health systems and the health workforce. It concludes by making a strong case for the building up and strengthening the health workforce, insisting on the crucial role of nurses, their training, and involvement in chronic disease management (Alexander Bischoff et al., 2009)

Global development assistance for health (DAH) was estimated at $21.8 billion in 2007. A recent assessment shows that official development assistance for health reached $26.4 billion in 2008, surpassing all prior years. No specific mention is made of DAH for NCDs in developing countries in either study. This paper aims to fill that gap. We conducted an analysis of donor spending on NCDs in developing countries from 2001 to 2008 that reveals that less than 3 percent ($503 million out of $22 billion) of overall DAH was dedicated to NCDs in 2007 (By Rachel Nugent and Andrea B. Feigl).

The Lancet Am 14. März 2015 hat die Fachzeitschrift "The Lancet" eine dreiteilige Artikelserie lanciert, um den Weg zu einer tabakfreien Welt aufzuzeigen. Um dieses Ziel zu erreichen, müsste sich China stärker engagieren, da jede/jeder dritte RaucherIn im Reich der Mitte lebt. Weiter müsste die Tabakindustrie in die Pflicht genommen werden, da diese den Restriktionen in der industrialisierten Welt damit begegnet, dass sie ihre Märkte in Schwellen- und Entwicklungsländern ausweitet. The UN High-Level Meeting (UN HLM) on NCDs in September, 2011, provides an unrivalled opportunity to create a sustained rights-based global movement to tackle NCDs, analogous to the UN General Assembly Special Session on HIV infection and AIDS a decade ago, which concluded that dealing with the disease was central to the development agenda. Political leadership at the highest level, with international coordination and consensus for priority actions and interventions are crucial responses to the crisis in NCDs and to facilitate national action (Robert Beaglehole et al., April 2011).

Mit diesem MMS-Bulletin möchten wir auf den weltweiten Tabakkonsum und seine Folgen für die Gesundheit von Mensch und Umwelt aufmerksam machen. Wir wollen die aggressiven Methoden der Tabakindustrie kritisch hinterfragen und den Einfluss von Tabak für die Erreichung der UNO-Nachhaltigkeitsziele (SDGs) aufzeigen. The Lancet As part of a Lancet's Series on Obesity the authors of this article propose a four step accountability framework for creating healty food environments and reducing global obesity. "The framework identifies multiple levers for change, including quasiregulatory and other approaches that involve government-specified and government-monitored progress of private sector performance, government procurement mechanisms, improved transparency, monitoring of actions, and management of conflicts of interest. Strengthened accountability systems would support government leadership and stewardship, constrain the influence of private sector actors with major conflicts of interest on public policy development, and reinforce the engagement of civil society in creating demand for healthy food environments and in monitoring progress towards obesity action objectives."

We know that awareness of chronic epidemic disease has increased over the past few years and this is illustrated by the formation of NCDnet, an initiative of the World Health Organisation (WHO) which is working in the area of non-communicable disease, and the development of a collaboration between eight Medical Research Councils (MRC) called the Global Alliance for Chronic Diseases (GACD). The next logical step for OxHA is to move from awareness raising to implementing community-based research, driving policy change and developing communications.

The NCD Alliance uses targeted advocacy and outreach to ensure that NCDs are recognized as a major cause of poverty, a barrier to economic development and a global emergency. This is done by working with a wide range of partners and organizations, speaking with a united voice at key international meetings, and pressing governments to recognize that NCDs are a global development priority requiring an urgent response. (…) "We are an informal alliance of four international federations (IDF, UICC, worldheart, The Union) representing the four main NCDs outlined in the World Health Organization’s 2008-2013 Action Plan for NCDs: cardiovascular disease, diabetes, cancer, and chronic respiratory disease."
