SolidarMed is the Swiss organisation for health in Africa and improves health care for 1.5 million people in Lesotho, Moçambique, Tanzania, Zambia and Zimbabwe. The health of mothers, pregnant women, newborns and children is a central concern and receives particular attention in the projects. In cooperation with local partner organisations, hospitals and villages, SolidarMed promotes help for self-help.

The humanitarian crisis in northern Mozambique is hitting the poorest of the poor.

SolidarMed "In Chiúre District, SolidarMed's project region in Mozambique, attacks by Non State Armed Groups have been flaring up since last December. Since then, the population is deprived from essential health care services. Many of those affected had fled from earlier attacks in the north of the district to the Chiúre region. Now they are on the move again, out of Chiúre, without water, without protection from the rain or sun and, of course, without medical care. - «Healthcare in the Chiúre district has completely collapsed because most of the health workers have fled.»

Working together to tackle diabetes

SolidarMed "Diabetes is one of the most common non-communicable diseases worldwide. In Zimbabwe and Lesotho, SolidarMed is working with the World Diabetes Foundation to provide access to diabetes prevention, diagnosis and treatment for people in rural regions. Diabetes causes around two million deaths a year, most of which are in low- and middle income countries. Particularly in rural regions of southern Africa, hospitals and health centres have until now been barely equipped to handle such non-communicable diseases, and so medical care and prevention are virtually non-existent."

In einem dreijährigen Projekt verbessert SolidarMed die Überlebenschancen von Frühgeborenen und untergewichtigen Neugeborenen in der ländlichen Region Morogoro und setzt dabei auf eine bewährte Methode. (...) Dank SolidarMed hat sich die Situation am Mahenge-Spital bereits stark verbessert: Das Distriktspital ist eines von drei Spitälern in der ländlichen Region Morogoro, deren Abteilungen für Neugeborene mit der Unterstützung von SolidarMed ausgebaut werden."

Diabetes oder Bluthochdruck belasten zunehmend die Gesundheitsversorgung im südlichen Afrika. Deshalb widmet sich SolidarMed in einer Zusammenarbeit mit dem Universitätsspital Basel dieser Problematik.

SolidarMed "Das mehrjährige ComBaCaL-Projekt soll die Krankheitslast durch nicht-übertragbare Krankheiten in Lesotho reduzieren. In einem ersten Schritt erfasste SolidarMed zusammen mit der klinischen Epidemiologie der Universität Basel die aktuelle Situation: Wie verbreitet sind Diabetes, Übergewicht, Rauchen oder Bluthochdruck in der Bergbevölkerung Lesothos tatsächlich? Über 6’000 Personen in den zwei Distrikten Mokhotlong und Butha-Buthe wurden dabei miteinbezogen. Noch nie wurden diese Zahlen in Lesotho bisher so systematisch erhoben. Die Resultate sind alarmierend. Bluthochdruck und Diabetes führen zu einer grossen, bisher wenig beachteten Krankheitslast in der Bevölkerung. Die Lebensqualität und Produktivität der Betroffenen ist bereits heute stark eingeschränkt."

SolidarMed "Ihrer Forschung zufolge verbinden die meisten Menschen hauptsächlich negative Dinge wie Armut, Hunger und Korruption mit Afrika. Ist das ein falsches Bild vom Kontinent? Es ist nicht grundlegend falsch, denn extreme Armut ist nach wie vor eine Realität für Millionen von Menschen dieser Welt, und überdurchschnittlich viele davon leben in Afrika südlich der Sahara. Dennoch ist ein per se negatives Bild von «Afrika» in zweifacher Hinsicht falsch."

Lucia Gonzáles arbeitet im Bereich «Forschung und Entwicklung». Mit ihrer internationalen Erfahrung als Ärztin und Forscherin wirkt sie in verschiedenen Projekten mit. Im Interview erzählt sie, warum Forschung für sie als Ärztin eine Pflicht ist.

SolidarMed "Lucia, warum hast du Medizin studiert? Das ist eine lustige Geschichte. Ich hatte nie daran gedacht, Medizinerin zu werden. Ich wollte grundlegende Wissenschaften wie z. B. Astrophysik studieren. Aber meine Mutter war Pflegefachfrau für Pädiatrie in einem Spital. Ein Oberarzt, der mit ihr zusammenarbeitete, gab den Auslöser für meinen Entscheid zum Medizinstudium: Er sagte zu mir: «Willst du die Welt sehen und in deinem Leben unabhängig sein? Dann studiere Medizin.» Ich habe in der Medizin meinen Traum gefunden."

Ganzheitliche Stärkung des Gesundheitssystems in Sambia

In enger Zusammenarbeit mit dem Gesundheitsministerium verbessert SolidarMed in Sambia die Ausbildung von Pflegepersonal und klinischen Ärzt:innen und macht mit der Schaffung von Wohnraum die Arbeit auch auf dem Land attraktiver.

SolidarMed "Es ist ein aussergewöhnlicher Tag für das SolidarMed-Team in Sambia. Vor dem frisch erstellten Ausbildungszentrum in Kafue steigt Sylvia Masebo, die Gesundheitsministerin von Sambia, aus dem Auto und schüttelt mit grosser Freude SolidarMeds Landeskoordinator John Tierney die Hand. Der Chor der Pflegefachkräfte singt und tanzt ihren Willkommenssong. «Die Eröffnung des ersten multidisziplinären Ausbildungszentrums in Sambia erfüllt uns mit Stolz und grosser Freude», sagt John Tierney zur Begrüssung. Ministerin Sylvia Masebo durchschneidet das rote Band und übergibt das Gebäude offiziell dem Distriktspital und der Pflegefachschule in Kafue zur Ausbildung von Studierenden und klinischem Personal."

SolidarMed "Landeskoordinatorin Barbara Kruspan lebt seit bald 30 Jahren in der Provinz Cabo Delgado. Seit 2017 treiben in den Küstenbezirken der nördlichen Provinz nicht-staatliche und schwer bewaffnete Gruppierungen ihr Unwesen. Dörfer werden überfallen, Menschen enthauptet. Die bereits zuvor von Wirbelstürmen, Hunger und schwacher Gesundheitsversorgung stark gebeutelte Bevölkerung leidet. Aufgrund der gewalttätigen Unruhen im Norden sind in der Provinz Cabo Delgado rund 1.5 Mio. Menschen auf humanitäre Hilfe angewiesen. Die Gesundheits- und Nahrungsmittelversorgung ist nicht mehr gewährleistet. Die Hälfte der geflohenen Kinder ist chronisch unterernährt."

From development project to locally-managed business

Since 2012, SolidarMed has been building houses for staff with the support of the Hilti Houndation to provide attractive accommodation at remote healthcare facilities.

SolidarMed "At many health centres and hospitals in rural Zambia, there is a lack of accommodation for healthcare workers and their families, which makes recruiting and retaining good medical staff a challenge. Since 2012, SolidarMed has been building homes for staff with the support of the Hilti Foundation so that rural healthcare facilities can also offer attractive housing for staff. In this way, SolidarMed is working to tackle the acute shortage of healthcare workers in rural areas. In 2024, the investment and development project is to become an independent social enterprise."

Die 57-jährige hat es sich zum Ziel gesetzt, in ihrem Umfeld möglichst vielen HIV-infizierten und armutsbetroffenen Kinder und Jugendlichen zu einem normalen Leben zu verhelfen. (...) Welche Art von Unterstützung brauchen Kinder und Jugendliche mit HIV? In erster Linie müssen sie ihre Medikamente zur Unterdrückung des Virus regelmässig einnehmen können und wollen. Manche sehen keinen Sinn darin, weil sie denken, ihr Leben habe mit HIV sowieso keine Zukunft."

SolidarMed "Ein bereits seit 2017 schwelender Konflikt im Norden von Mosambik ist mehrmals unbeachtet der Öffentlichkeit eskaliert. Mittlerweile haben sich über 800'000 Menschen weiter im Süden niedergelassen. Da in der Region grosse Armut herrscht, ist nicht nur das bereits sehr schwache Gesundheitssystem komplett überlastet, sondern auch der gesamte Druck auf die lokale Bevölkerung hoch. Obwohl die Region grosse Rohstoffschätze aufweist, ist die humanitäre Not gross. Seit 2017 treiben in den nördlichen und zentralen Küstenbezirken Cabo Delgados nicht-staatliche und schwer bewaffnete Gruppierungen ihr Unwesen. Dörfer werden überfallen, Menschen enthauptet. Laut der Konflikt-Beobachtungsstelle ACLED wurden seit 2017 mehr als 4’000 Menschen Opfer dieser Gewaltakte. Die bereits zuvor von Wirbelstürmen, Hunger und schwacher Gesundheitsversorgung stark gebeutelte Bevölkerung leidet."

Joint project called ComBaCaL (Community-Based Chronic Care Lesotho) between SolidarMed, the Swiss TPH, the MoH in Lesotho, the University of Zurich and the National University of Lesotho.

Survive and thrive – a colour-coded triage system

SolidarMed "Waiting times are often long in the accident and emergency departments in northern Mozambique. Thanks to support from SolidarMed, children are now given a coloured card on arrival at four healthcare facilities. This means that those in life-threatening situations are treated more quickly than less urgent cases and therefore have better chances of survival than before. Child mortality in Mozambique remains high, with 74 in every 1,000 children not seeing their fifth birthday. By way of comparison, in Switzerland that figure is four in 1,000. The main cause of death in children in Mozambique is malaria. Particularly for children with pre-existing health problems, such as anaemia or malnutrition, a malaria infection can quickly become life-threatening."

SolidarMed "Eine Ambulanz, die mit Blaulicht und Sirene zu einem Verkehrsunfall eilt. Ein Team von qualifizierten Fachkräften, das in der Notaufnahme bereitsteht, mit ausreichend Verbrauchsmaterial, Blutkonserven und Medikamenten – und einer zuverlässigen Stromversorgung. Wir wissen es: Eine rasche Behandlung im Notfall kann über Leben oder Tod entscheiden. In den ländlichen und teilweise schwer zugänglichen Gebieten im Norden von Mosambik entspricht ein solches Szenario noch einem Wunschdenken. (...) Damit Patient:innen möglichst schnell und günstig ein Spital erreichen, führt SolidarMed seit 2017 ein Projekt mit Ambulanztaxis durch, das im Distrikt Chiúre schon gute Resultate bringt. Doch es sind nicht nur die schlechten Strassen, die ausserordentlich grossen Distanzen und fehlenden Mittel, die in einem Notfall zum Verhängnis werden können. Eine weitere Barriere ist fehlendes Wissen: In den Dörfern sind sich Patient:innen und ihre Familien der Dringlichkeit oft nicht bewusst und suchen nicht schnell genug Hilfe."

SolidarMed "An was denken Sie, wenn Sie den Begriff «Afrika» hören? Welche Bilder tauchen in Ihrem Kopf auf? Sind es Hunger, Armut und Krankheit? Unterernährte Kinder, trockene Felder, staubige Hütten? Damit wären Sie nicht allein, denn unsere Vorstellung von Afrika ist geprägt von Bildern und Schlagzeilen aus den Medien. Doch stimmt das so? Katastrophen, Klimawandel und Kindersterblichkeit – es gibt sie alle, und ihre Folgen sind schwer zu ertragen. Und doch ist Afrika so viel mehr: gelebte Hoffnung, Aufbruch und Energie! Enfants du Monde À l’occasion de la Journée mondiale de la santé, Enfants du Monde dévoile un nouveau projet visant à améliorer la santé sexuelle des adolescent·es en Tanzanie, en collaboration avec l’ONG SolidarMed et les Ministères de la santé et de l’éducation tanzaniens. (...) Le défi de la santé sexuelle dans un pays jeune La population de la Tanzanie est estimée aujourd’hui à 61 millions d'habitant·es, dont près d’un quart sont des adolescent·es de 10 à 19 ans. La bonne santé de ce groupe d’âge est étroitement liée à leur santé sexuelle et reproductive, notamment les infections sexuellement transmissibles et les grossesses précoces. Or, parmi les adolescent·es sexuellement actif·ves, la moitié seulement a utilisé un préservatif lors de leur dernier rapport.

Gaining a perspective thanks to vocational training

In Hyderabad, India, young people affected by poverty and HIV can complete vocational training courses thanks to SolidarMed. This way, they become financially independent, can support their families and escape the stigma of poverty and HIV/Aids.

SolidarMed On 12 January, around 200 adolescents and young adults received their diplomas in Hyderabad, India. They have all successfully completed a SolidarMed vocational training course and now have good chances of entering the labour market. Generally, around nine out of ten graduates already have a job on the last day of the course or have decided to become self-employed. Unfortunately, it cannot be taken for granted that young people affected by HIV and poverty in India have a chance of a financially stable future. This is because the stigma surrounding HIV and AIDS is great in India: HIV-positive people are excluded from the family and society, and children are often prevented from attending school when their parents' HIV status becomes known.

Improving HIV treatment in rural Lesotho

SoldiarMed SolidarMed and Swiss TPH jointly introduced a new testing procedure in rural Lesotho in 2021. It enables to improve health care for people affected by HIV in rural areas. The SolidarMed-supported laboratory is the first to date to use such tests in HIV treatment in rural Lesotho. Treatment of HIV requires good adherence to therapy to provide optimal control of the virus. If, based on the condition of the person affected by HIV, the attending physician is concerned that the current medication may not be the right one, genotypic HIV resistance testing is used to quickly detect virologic failure or resistance to treatments.

Health care on wheels

The four-wheeled clinic brings health care to Lesotho's most remote villages. An enormous step forward for the medically underserved mountain population with the second highest HIV rate worldwide

SolidarMed This mobile practice is the result of a joint project between SolidarMed and the Canadian organization Bracelet of Hope. Launched in March 2020, the mobile practice is a unique initiative in Lesotho, the very mountainous country in southern Africa affectionately known as the "Kingdom in the Sky." More than 40 percent of the population lives in remote and hard-to-reach villages, and 57 percent live below the poverty line. SolidarMed Lesotho received the Johnson Johnson vaccine in July. The health management team in Butha-Buthe conducted the Covid-19 vaccination campaign in the villages to reach the rural population in this way. SolidarMed was one of the main supporters of this initiative and provided personnel support and vehicles.

Neben einem umfassenden COVID-19-Nothilfeprogramm konnten die laufenden und geplanten Programme trotz Krise erfolgreich umgesetzt werden

SolidarMed 2,78 Millionen Menschen profitierten dank SolidarMed im Jahr 2021 von einer verbesserten Gesundheitsversorgung. Der Jahresbericht 2020 zeigt auf wie neben einem umfassenden COVID-19-Nothilfeprogramm unter dem Motto «Leave no one behind» auch die regulären Programme trotz Krise erfolgreich umgesetzt wurden. SolidarMed, die Schweizer Organisation für Gesundheit in Afrika mit Sitz in Luzern und Teams im östlichen und südlichen Afrika, blickt trotz Pandemie und Krise auf ein sehr erfolgreiches 2020 zurück. Knapp 2,8 Millionen Menschen erhielten mit Unterstützung der Organisation eine verbesserte Gesundheitsversorgung in Kenia, Lesotho, Mosambik, Tansania, Sambia, Simbabwe, Südafrika und Indien.

Development of a power-free cold box

SolidarMed In close collaboration with EssentialTech and the Laboratory for Functional Inorganic Materials (LFIM) at the Swiss Federal Institute of Technology Lausanne (EPFL), SolidarMed is developing a cold box over the next two years that does not require electricity and can maintain a temperature of 2 to 8 °C for weeks. In low-income areas, the lack of a continuous cold chain, for example due to an unstable electricity grid, means that vaccine doses have to be disposed of. Every year, 25% of vaccinations have to be disposed of for this reason. Every year, 1.5 million children under the age of five die due to lack of access to vaccinations. This situation is unacceptable.

Der Kampf um die Stadt erschwert die zuvor schon herausfordernde Lage in der Region. Landeskoordinatorin Barbara Kruspan lebt seit bald 30 Jahren in der Provinz Cabo Delgado und ist seit knapp vier Jahren für SolidarMed tätig. Früher hat sie in Mocímboa da Praia gelebt – dort wo jetzt islamistisch-motivierte Terroristen ihr Unwesen treiben. Sie kennt die Situation sehr gut. The situation in the landlocked country of Lesotho is also tense: There has been an extreme increase in corona cases there since mid-December. In the beginning of January, hospitals as well as the mortuaries were overloaded.

SolidarMed Non-Communicable Diseases (NCDs) such as diabetes, high blood pressure and impaired mental health are the number one cause of death and disability globally. More than 75% of all premature NCD deaths occur in low-and middle-income countries. A consortium of implementation partners and researchers led by the Swiss NGO SolidarMed proposes a multi-disciplinary research for development program fighting this disproportionate health and socio-economic burden. The Swiss Agency for Development and Cooperation (SDC)will fund the program with 6 Mio CHF over the next five years.

Mit der Übertragung der Projekte und des Vermögens von Aids & Kind an SolidarMed erhalten Kinder und Jugendliche mit HIV/Aids in Südafrika, Kenia und Indien eine sichere Perspektive. Die bisherigen Projekte werden gemäss dem Stiftungszweck von Aids & Kind weitergeführt und der verantwortliche Projektleiter und Co-Geschäftsführer wechselt zu SolidarMed. SolidarMed Die Entwicklungshilfe verändert sich. Neben der klassischen Hilfe, etablieren sich immer mehr auch neue Formen von Hilfe. SolidarMed investierte die letzten Jahre mit der notwendigen Ausdauer in das medizinische Angebot der Gesundheitszentren Chiúres, im Norden Mosambiks. Um davon zu profitieren, müssen die Patient/innen rechtzeitig im Gesundheitszentrum ankommen. Momentan gibt es im ganzen Distrikt jedoch nur eine Ambulanz für die rund 200'000 Menschen. SolidarMed spürt mit der Tuk-Tuk-Ambulanz einer spannenden Lösung nach, die langfristig in Mosambik funktionieren kann. SolidarMed Zwei Dosen Insulin täglich und sehr viel Selbstdisziplin erlauben der 25-jährigen Salvina heute ein fast normales Leben. Dank einer genialen Erfindung überlebt sie trotz ihrer Zuckerkrankheit auch unter extremen Bedingungen. (...) Salvina Kilunda ist heute 25 Jahre alt. An den meisten Orten in Afrika südlich der Sahara wäre sie bereits tot. In ihrer Pubertät traten immer stärkere Schmerzen in den Beinen auf. Darmbeschwerden häuften sich. Sie verlor an Gewicht. Als sie schliesslich das Spital in Lugala aufsuchte, war sie nur noch Haut und Knochen. Die Diagnose: eine Typ–1 Kinderdiabetes. SolidarMed - BMC Infectious Diseases The SESOTHO Trial aims to answer this question by comparing WHO guidelines for switching patients with alternate guidelines of lower thresholds, to improve outcomes and decrease the risk of resistant mutations. The trial is set up by the consortium composed of SolidarMed, SwissTPH, University Hospital Basel, Ministry of Health of Lesotho and the Department of Biomedicine, University of Basel, who’s main aim is to support the Lesotho government to reach the 90-90-90 UNAIDS Targets by 2020. SolidarMed, SRC With the Millennium Development goals coming to an end by 2015, a debate is going on about what could be a coherent global framework for the upcoming decade. As it stands now, the new framework will be called “sustainable development goals” and build on the triple bottom line: Social inclusion, economic development and ecologic sustainability. Concerning health, various concepts have been discussed: Wellbeing; reduction of avoidable mortality/morbidity, universal health coverage (UHC) and others. Amongst these, UHC has emerged as the most promising. To discuss and clarify the terminology and meaning of UHC, the Swiss Red Cross and SolidarMed have held a joint workshop in 2013. Out of this workshop, we have now developed a joint, short paper, which is now available.
