Es steht vor der Tür – das MMS Symposium vom 7. November 2018 zur Zukunft der Basisgesundheitsversorgung. Eine Vorschau auf eine brisante Tagung.
Revolutionen im Gesundheitsbereich sind gekommen und verschwunden – warum diese nicht? Warum mobilisiert die Alma-Ata-Deklaration zur Basisgesundheitsversorgung auch vierzig Jahre nach der Konferenz noch immer Menschen weltweit? Ich vermute, weil es den Anspruch auf das grundlegende Recht auf Gesundheit fassbar und ein immer technologischeres System menschlich macht.
Die Basisgesundheitsversorgung verlangt nach Mitsprache der Gemeinden und Gemeinschaften zur Gestaltung ihrer alltäglichen Gesundheitsversorgung und eines Umfeldes, in dem sie gesund bleiben können. Sie ist der direkte Zugang zu einem als komplex und oft auch als entmenschlicht empfundenen System. Sie enthält das Versprechen und den Anspruch, dass die Gesundheitsversorgung den Mensch ins Zentrum stellt. Und sie verlangt, dass das System demokratisch und damit bedarfs- und bedürfnisgerecht kontrolliert und gesteuert wird. Das Konzept der Basisgesundheitsversorgung ist damit Kristallisationspunkt, um das Recht auf Gesundheit zu verwirklichen.
Diesen Kristallisationspunkt bildet auch das diesjährige MMS Symposium, bei dem es exakt um die Zukunft der Basisgesundheitsversorgung und um ihre Rolle in der internationalen Gesundheitszusammenarbeit geht. Es bringt die in der Schweiz für das Recht auf Gesundheit engagierten Menschen zusammen. Das Netzwerk Medicus Mundi Schweiz wird ihr Manifest Gesundheit für alle in einer Generation vorstellen und diskutieren. Und wir tun dies nicht allein und wir tun dies auch selbstkritisch, um bereit zu sein für die anstehenden Auseinandersetzungen um die Agenda 2030 der vereinten Nationen – also von uns allen in der Schweiz und weltweit zum Durchbruch zu verhelfen.
An unserem Symposium erwarten wir die Nationalratsvizepräsidentin Marina Carobbio Guscetti. Und wenn sie über „The future of Primary Health Care: Contradictions between Global Challenges and Political Decisions” referiert, dann weiss sie wahrlich, wovon sie spricht: Sie ist Präsidentin von AMCA und damit mit der internationalen Gesundheitszusammenarbeit bestens vertraut. Und sie ist Hausärztin im Kanton Tessin – sie kennt die Bedeutung der Basisgesundheitsversorgung ganz direkt aus ihrer Arbeit.
An unserem Symposium wird auch Francisco Songane mit uns sein. Der ehemalige Gesundheitsminister von Mosambik kennt aus jahrelangem Engagement die Stolpersteine und die aktiven Widerstände zur Erreichung des Rechts auf Gesundheit für alle. Und wir erwarten Gäste aus Chiapas, aus Moldavien, aus Italien und der Schweiz.

"In 2008, WHO launched the final report of the Commission on Social Determinants of Health (CSDH) that concluded “social injustice is killing people on a grand scale”. A decade later, how should we judge the CSDH's impact? A Google search for the CSDH yields 156 000 results and the accompanying Lancet paper has had 932 citations. The CSDH led to two World Health Assembly resolutions and more than 100 countries adopted the Rio Political Declaration on Social Determinants of Health in 2011. The CSDH's report has become a foundational text for how crucial social determinants are to health and health equity. And yet, there has not been widespread policy uptake of its recommendations to improve daily living conditions, tackle the inequitable distribution of power, money, and resources, and monitor both inequities and the impact of policies to address them. Instead, although a number of countries actively engaged with the CSDH's ideas and gave policy consideration to social determinants, a revival of austerity policies harmed health and health equity, with stagnating life expectancy and widening mortality gaps in some countries." UN High-Level Meeting zu nichtübertragbaren Krankheiten
Die mit Spannung erwartete Generalversammlung der Vereinten Nationen, um über Massnahmen gegen nichtübertragbare Krankheiten (NCDs) zu beraten, ist nach Ansicht vieler zivilgesellschaftlicher Organisationen erneut eine vertane Gelegenheit, um die globale Epidemie der NCDs in den Griff zu bekommen. In einem offenen Brief äussern sie sich kritisch gegenüber der verabschiedeten Erklärung: Neben völlig unzureichenden Finanzierungsmassnahmen übernehmen die Regierungen auch politisch zuwenig Verantwortung. Die Zuckersteuer wurde völlig aus der Erklärung gestrichen und auch sonst gibt es kaum konktrete Massnahmen, um die Tabak- und Lebensmittelindustrie angemessen zu besteuern. In einem offenen Brief haben sich schweizer und norwegische NGOs, darunter auch das Netzwerk Medicus Mundi Schweiz an ihre jeweiligen Handelsminister gewandt, mit der Forderung, die Verhandlungen zum TRIPS-plus Abkommen mit Indonesien sofort zu stoppen.

Obwohl sich die gesundheitliche Versorgung in Ländern mit niedrigem und mittlerem Einkommen in den letzten Jahrzehnten verbessert hat, wird die Mehrheit der Gesundheitssyteme den neuen Anforderungen, gestiegenen Erwartungen der Bevölkerung und den veränderten Gesundheitsbedürfnissen in keiner Weise gerecht.

An der diesjährigen Fachtagung des Netzwerks Medicus Mundi Schweiz (MMS) in Bern waren sich die Fachleute einig: Geschlechtsspezifische Gewalt (Gender-based Violence, GBV) muss auf allen Ebenen bekämpft werden. Die #MeToo-Bewegung hat ihre Verbreitung in allen Gesellschaftsschichten an den Tag gebracht. Doch in vielen Ländern bleibt die Gewalt im Verborgenen. Sie wird von staatlicher Seite zu wenig bekämpft und die juristischen Strukturen zum Schutz der Opfer sind zu schwach. Die Fachtagung zog weitere Lehren und Erkenntnisse, und unterstützte den Austausch und das gegenseitige Lernen - über eine Thematik, die keine einfachen Lösungen bietet. Medicus Mundi International (MMI) Astana, 25-26 October 2018. The world will come together in Astana, Kazakhstan at the Global Conference on Primary Health Care "to renew a commitment to primary health care to achieve universal health coverage and the Sustainable Development Goals". The MMI Network, an NGO in official relations with WHO, is invited to the conference and will attend it with a small delegation, as part of the civil society constituency. We are currently exploring with civil society colleagues about how to guarantee civil society representation in the conference and how to broaden the space for civil society in the formal conference programme. We are happy to confirm that the following proposals are accepted by the conference organizers: CAFÉ SESSION on "Calling for a New Global Economic Order - the forgotten element of the Alma-Ata Declaration". Reacting to a call by the conference organizers, the MMI Network sucessully submitted a proposal for a "café session" on "Calling for a New Global Economic Order - the forgotten element of the Alma-Ata Declaration". Institute of Tropical Medicine (ITM) Antwerp, 15-17 October 2018, International workshop at the Institute of Tropical Medicine (ITM). "Many low- and middle-income countries (LMICs) have signaled their intention to be part of the global agenda for Universal Health Coverage (UHC). The WHO and others have set the course: as an international community, we have a good understanding of the objectives to be achieved, the indicators to follow, the things not to do and the possible options. This ‘global’ knowledge does not, however, exhaust all the questions that national and international actors may have at country level. Questions are multiple and to a large extent are, and will remain, peculiar to the country’s situation. To move towards UHC, each country will have to find its own way. In a recent article published by members of the organising committee, it was noted that this indicates that self-reliant learning will be, alongside national leadership, the most critical capacity for success in UHC. Elle aura pour thème « Les tribunaux d’arbitrage internationaux privés: un danger pour la souveraineté des peuples » et se tiendra le mardi 16 octobre 2018 à 19h30 à la Maison des associations à la rue des Savoises 15. La pollution en Amazonie équatorienne par les opérations pétrolières de Chevron (avant Texaco) compte parmi les plus importantes du monde. Il affecte l’une des régions les plus riches en biodiversité et a impacté gravement la santé de ses habitants. Depuis plus de 25 ans, 30 000 victimes mènent des procès dans plusieurs pays et devant des instances internationales pour obtenir la réparation des dommages.
medico international schweiz Im Vorfeld der Wahlen Anfang Juli 2018 ist das Ausmass der Gewalt in Mexiko drastisch angestiegen. Die Anzahl Mordfälle für das laufende Jahr überstieg bereits im Juli die Zahlen der letzten 20 Jahre. Stark davon betroffen sind auch MenschenrechtsverteidigerInnen. Die hohe Straflosigkeit im Land führt dazu, dass Menschenrechtsverletzungen wie gewaltsames Verschwindenlassen, aussergerichtliche Hinrichtungen, sexuelle Gewalt und Landenteignungen kein Ende nehmen. Dies in einem Kontext, in dem Drogenkartelle immer mehr Macht ausüben, der Einfluss des Militärs wächst und Unternehmen umstrittene Megaprojekte umsetzen, die zu Konflikten führen. Die Diskussionsrunde thematisiert die Gewalteskalation im Rahmen der Wahlen, die Perspektiven für die Menschenrechte unter der neuen Regierung und die Rolle der Schweiz.
Women's Hope International Der Film zeigt Daw Nweni Cho, eine staatliche Hebamme in Myanmar, bei ihrer täglichen Arbeit. Daw Nweni Cho versorgt sieben Dörfer mit über 2000 Menschen und ist neben ihren Aufgaben als Hebamme auch für die medizinische Grundversorgung der Dorfbewohner zuständig. Auf eindrückliche Weise veranschaulicht der Film, unter welchen Bedingungen vom Staat angestellte Hebammen in Myanmar arbeiten sowie die Umstände, in denen Frauen gebären und welchen Risiken sie dabei ausgesetzt sind. The anniversary of this seminal milestone in global health provides an opportunity to reaffirm the principles of the original declaration and underline the importance of primary health care to achieve our collective goals for the 21st century.

MMS Ein Dokumentarfilm über den turbulenten Weg Mosambiks im Aufbau seines öffentlichen Gesundheitssystems. "La Luta Continua" ("Der Kampf geht weiter") wurde von Medicus Mundi Catalunya und Kanaki Films 2014 produziert und befasst sich mit den Fortschritten, den Herausforderungen und Schwierigkeiten, die in Mosambik aufgetreten sind, um ein Gesundheitssystem für alle zu schaffen. Dabei nimmt der Film unterschiedliche Perspektiven ein, mit Interviews mit Funktionären und Beamten des Gesundheitsministeriums, Gebern, internationalen Organisationen und mit Vertretern der Zivilgesellschaft. Sie alle behaupten, edle Absichten zu haben, die Gesundheit der Bevölkerung zu verbessern. Mit einer Laufzeit von 40 Minuten zeigt "A Luta Continua" die Umstände und Tendenzen des Basisgesundheitskonzept (PHC) basierend auf der Alma-Ata Deklaration, weit entfernt von ihrem revolutionären ursprünglichen Inhalt.
MMS Gesundheit ist ein grundlegendes Menschenrecht. Dennoch sorgen Ungleichheit, Armut, Ausbeutung, Gewalt und Ungerechtigkeit dafür, dass bis heute einer Milliarde Menschen der Zugang zu Gesundheitsversorgung verwehrt bleibt. Um das Ziel "Gesundheit für alle" zu erreichen, müssen Ungleichheiten beseitigt und Ressourcen besser verteilt werden, darf niemand ausgeschlossen werden und müssen politische und wirtschaftliche Interessen darauf ausgerichtet werden, "Gesundheit für alle" zu verwirklichen. Angesichts des 40. Jubiläums der Erklärung von Alma Ata können wir klar feststellen, dass die Vision von Alma-Ata dringender umgesetzt werden muss denn je. Medicus Mundi International (in) Mozambique "We want Mozambique to become a country of reference in championing Primary Health Care and to do so with scientific rigor, promoting international events for reflection and debate on how to improve PHC in the country. (...) To date, health research in Mozambique has been almost exclusively biomedical. However, we understand that the country faces challenges that go beyond this approach, and we cannot improve individual and collective health without a better understanding of the determinants that affect it. SDH can become an ideal theoretical and practical framework to reduce health inequities, particularly in Mozambique. It is in this context that medicusmundi will organize the First International Conference on Social Determinants of Health in Maputo."
Health Systems Global The Alma Ata vision of ‘Health for All’ remains as compelling today as it was in 1978, as reflected in goal 3 of the Sustainable Development Goals (SDGs). But the world has changed in forty years. Despite improved health outcomes, there remain extraordinary challenges for health equity and social inclusion, such as demographic and disease transitions, conflicts and their subsequent migrations, pluralistic health systems and markets, and climate change. Political systems still marginalize those most in need. Yet there are new opportunities for health systems to achieve universal coverage. The Fifth Global Symposium will advance conversations and collaborations on new ways of financing health; delivering services; and engaging the health workforce, new social and political alliances, and new applications of technologies to promote health for all.
Swiss TPH The course introduces the basic principles of strategic programme and project design and management, taking into consideration local, district, national and international/global evidence and realities and health system strengthening needs. It focuses on priority setting and practical aspects of the planning and implementation process, through the use of a simulation exercise and computer based support. Designed as a refresher course for professionals working in the field of programme and project planning, design and evaluation, the course is also open to participants with a DAS Health Care and Management in Tropical Countries (or equivalent) and is an optional module within the joint Master of Advanced Study in International Health.
Swiss TPH Health in detention is a one-week course offered by Swiss Tropical and Public Health Institute (Swiss TPH), an associated institute of the University of Basel, in collaboration with the International Committee of the Red Cross (ICRC). This course is open to participants working in the field of health in detention, nationally or internationally, and uses a multi-disciplinary approach to bring together academia, clinical, public health and legal experts.

Ein Meilenstein für die Internationale Zusammenarbeit in Gesundheit jährt sich im Herbst zum vierzigsten Mal. 1978 wurde an einer WHO-Konferenz in Alma Ata (Kasachstan) erstmals das Recht auf Gesundheit für alle postuliert. Eine Einschätzung aus Sicht des SRK. (...) «Engagement für das Recht auf Gesundheit braucht es heute mehr denn je», sind Thomas Gass und Vreni Wenger überzeugt: «Die 400 Millionen Menschen, die heute keinen Zugang zu Gesundheitsgrundversorgung haben, haben das Recht auf Gesundheit. Medienmitteilung: Safe Abortion Day 28.9.2018
Am Safe Abortion Day lenkt SEXUELLE GESUNDHEIT Schweiz die politische und öffentliche Aufmerksamkeit auf das Thema Schwangerschaftsabbruch, um an die Bedeutung des Zugangs zum Schwangerschaftsabbruch unter sicheren und geschützten Bedingungen zu erinnern. Ebenso haben alle Personen das Recht auf vertrauliche und ergebnisoffene psychosoziale Beratung und Begleitung. Weltweit werden jährlich rund 57 Millionen Schwangerschaftsabbrüche durchgeführt, fast die Hälfte davon unter unsicheren medizinischen Bedingungen. Die Mehrheit (97%) der unsicheren Abbrüche geschehen in Afrika, Asien und Lateinamerika.
La Centrale Sanitaire Suisse Romande était présente le samedi 22 septembre au Parc des Bastions (Genève) lors du Festival Alternatiba Léman 2018, aux côtés d’autres organisations et associations du bassin lémanique engagées pour le climat et la solidarité internationale. A travers divers stands, restaurations bios, films et autres animations pour enfants, le festival visait à contribuer à la lutte contre le réchauffement climatique au niveau local et international, à sensibiliser l’opinion publique, proposer des alternatives, à lutter contre le manque d’équité, et à s’impliquer pour un monde plus solidaire et pour la réduction des inégalités.
Ce dernier se penche sur la situation toujours dramatique de la Palestine et des territoires occupés, où la CSSR a des projets de réhabilitation de la santé mentale des enfants ex-détenus depuis de nombreuses années.
Bombenangriffe auf Zivilisten
HI ruft 4.500 Parlamentarier dazu auf, sich gegen Bombenangriffe auf Wohngebiete einzusetzen – eine Praxis, die in aktuellen Konflikten systematisch geworden ist und zu 92 Prozent Zivilisten trifft und tötet oder verletzt. Die politischen Entscheidungsträger müssen aktiv werden und dieser Praxis ein Ende setzen. Zu diesem Zweck fordert HI von den Parlamentariern in acht Ländern, Stellung zu beziehen, indem sie die Petition ‘Stop Bombing Civilians’ unterzeichnen und Druck auf ihre Regierungen auszuüben, um ein Ende von Explosivwaffen in bewohnten Gebieten voranzutreiben. MMS Ein Dokumentarfilm über den turbulenten Weg Mosambiks im Aufbau seines öffentlichen Gesundheitssystems. "La Luta Continua" ("Der Kampf geht weiter") wurde von Medicus Mundi Catalunya und Kanaki Films 2014 produziert und befasst sich mit den Fortschritten, den Herausforderungen und Schwierigkeiten, die in Mosambik aufgetreten sind, um ein Gesundheitssystem für alle zu schaffen. Dabei nimmt der Film unterschiedliche Perspektiven ein, mit Interviews mit Funktionären und Beamten des Gesundheitsministeriums, Gebern, internationalen Organisationen und mit Vertretern der Zivilgesellschaft. Sie alle behaupten, edle Absichten zu haben, die Gesundheit der Bevölkerung zu verbessern. Mit einer Laufzeit von 40 Minuten zeigt "A Luta Continua" die Umstände und Tendenzen des Basisgesundheitskonzept (PHC) basierend auf der Alma-Ata Deklaration, weit entfernt von ihrem revolutionären ursprünglichen Inhalt.
MMS Gesundheit ist ein grundlegendes Menschenrecht. Dennoch sorgen Ungleichheit, Armut, Ausbeutung, Gewalt und Ungerechtigkeit dafür, dass bis heute einer Milliarde Menschen der Zugang zu Gesundheitsversorgung verwehrt bleibt. Um das Ziel "Gesundheit für alle" zu erreichen, müssen Ungleichheiten beseitigt und Ressourcen besser verteilt werden, darf niemand ausgeschlossen werden und müssen politische und wirtschaftliche Interessen darauf ausgerichtet werden, "Gesundheit für alle" zu verwirklichen. Angesichts des 40. Jubiläums der Erklärung von Alma Ata können wir klar feststellen, dass die Vision von Alma-Ata dringender umgesetzt werden muss denn je.
